This position assists with rapid re-housing for individuals experiencing homelessness or those at risk of homelessness. Client services include liaising with landlords including social housing, support in acquiring ID, and finding rental housing. The Community Health Facilitator provides short-term ongoing support to promote successful tenancies and individual well-being. Position Summary Under the direction of the Program Manager, the Community Health Facilitator is, in accordance with professional practice standards, responsible for the following: Facilitate the intake of new clients, including program eligibility, initial backssment, and assignment of appropriate team resources, including outreach workers Identify and access appropriate resources for clients outside the team/MCC as appropriate Coordinate program activities with the team as directed Provide competent services MCC to clients in accordance with required standards, legal requirements, evidence-informed practices and policies Provide intake, backssment and.
