QUALIFICATIONS Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, psychology, or a related human service field 2 years of supervising experience Knowledge of community programs, services, evidence-based practices, and various treatment modalities used to work with youth and their families to navigate child-serving systems Ability to actively participate in a fast-paced, multidisciplinary team environment Valid driver's license and proof of vehicle insurance Strong organizational and time management abilities Excellent interpersonal and written communication skills WHAT TO EXPECT AS AN INTENSIVE CARE COORDINATION SPECIALIST As an Intensive Care Coordination Specialist, you will be the cornerstone of our Care Coordination and Support program. Your responsibilities include evaluating and interpreting referral information, conducting strength needs and cultural diversity assessments, developing Crisis Prevention and Crisis Safety plans, facilitating regular Child and Family Team meetings, and providing linkage to in-network providers. Engage with families through face-to-face visits, ensuring the delivery of strength-based, family-centered, culturally competent, and trauma-informed services. Your role is crucial in helping families and youth achieve their goals within the Pathways to Success framework!
